vibrating or shuddering

I've noticed this happen a few times but not always. Anyway, when I am doing say 30 and in 4th gear and begin to accelerate, as the revs hit around 2K, the whole car vibrates and shudders very violently (no exaggeration, it's also pretty noisy too) for about 500K's worth of revs then settles...
